If you go to your "My Ebay" page (upper right hand corner of screen)
-- there is a drop down menu next to the item that you have not
received. You can "report an item not received"

Default Insurance vs. Liability

On Ebay a seller can make any rules they want as long as they state
them in their ad. I usually put "Canadian buyers add $5 for S&H" in my
auctions for example.
