need help with consignment contract

I need a little help writing a consignment contract for my jewelry. Does
anyone have one that I could use. I have searched for a sample contract on
line and can't find one. I want to protect myself against theft, damage and
or loss.

I don't have a consignment contract but I am a contracts manager and would
review anything you come up with for loop holes or whatever.
