Spiral phase plate looking for advise on making shape..tia sal

Greetings all
I'm trying to make a spiral phase plate out of plastic (see link below). I have all the tools I'm just a little lost as
how I should procede. It doesn't have to be perfect or large. The Diameter would be just 5 inches

You going to have to grind perfectly flat radii that are individually each
different thickness. If you have the tools to grind plastic perfectly
parallel to the back with micrometer accuracy, then mount the plastic disk
to a bolt in the center, grind a radius, rotate, adjust, grind, rotate,
adjust, repeat for about 360 degrees.
If you don't understand how the plate works and don't have the precision
tools, then you can't make one that will work.
